If you are concerned about under-eye dark circles, please feel free to consult us first.
Severe under-eye unevenness can emphasize dark circles. Bulging occurs when orbital fat shifts forward or the skin under the eyes sags. Strong grooves, also called “Gorgo lines,” are another factor, influenced by lower eyelid ligaments.
These concerns may be resolved with theTransconjunctival Orbital Fat Repositioning.
A procedure performed from the inner eyelid (conjunctiva) to correct orbital fat displacement and Gorgo lines simultaneously.
Our Transconjunctival Orbital Fat Repositioning Evo adds CPF pre-layer fascia to cover the protruding orbital fat and prevent recurrence. This method combines natural aesthetics with functional beauty and has a wide range of applications.
The conjunctiva is incised, so no scars remain on the skin surface.
Protruding orbital fat is repositioned to under-eye hollows, correcting both issues simultaneously.
Unlike simple fat removal, this method includes measures to prevent recurrence, providing long-term effects.
The Transconjunctival Hamra procedure can perform recurrence-preventive treatments that simple fat removal cannot achieve. While it cannot stop the natural aging process under the eyes, taking measures to prevent recurrence allows for long-term results.
Those who want to correct under-eye sagging or Gorgo lines.
Those who do not want surface scars
Those seeking long-term results

Kuma CLI is a clinic with proven experience in Transconjunctival Orbital Fat Repositioning.
CPF round-type repositioning enhances recurrence prevention.
Micro-scar method reduces incision size and visibility.
We offer peace of mind with the industry’s longest 5-year guarantee.
Even after treatment, orbital fat displacement may recur. Using CPF pre-layer fascia, the exit causing hernia is sealed.
In general, surgeons often make a large incision in the conjunctiva to secure a better view. At our clinic, we limit the incision to about 3/5–2/3 the size used at other clinics.
Because the incision is smaller, not only is the scar less noticeable after surgery, but the risk of complications is also reduced.
*Please note that this is an average; individual results may vary.
We are extremely confident in the results of our procedure, which is why we offer a 5-year recurrence guarantee.
At Kuma CLI, we perform CPF round-type repositioning, taking particular care to prevent recurrence.
That is why we have established a 5-year guarantee system. In fact, we believe the preventive effects can last well beyond five years.
*This guarantee covers the recurrence of orbital fat hernia. It applies if the doctor determines that a recurrence has occurred. Any necessary treatment, including local anesthesia, will be provided free of charge.

FLOW
After the design is drawn, we will explain the design and the treatment flow for the day.
After confirming pain relief and sedation, a small incision (about 1.5 cm) is made inside the lower eyelid to begin treatment.
View of the eye area from the side
The space between the orbicularis oculi muscle and orbital fat is dissected.
The maxilla, orbicularis oculi muscle, and sub-orbicularis oculi fat are separated, along with the ligaments, to create a pocket for repositioning the orbital fat.
The septum is opened, and an orbital fat flap plus a septum-CPF anterior leaf composite flap is prepared.
Orbital fat is shifted beneath the dissected ligament and fixed with absorbable sutures. Eventually, the tissue adheres in place as the sutures dissolve.
The septum–CPF anterior leaf composite flap is then sutured along the orbital rim. This is the key to preventing recurrence. (Side view illustration)
The same flap is sutured along the orbital rim, shown here from the front. Again, this is the crucial step for recurrence prevention. (Front view illustration)
After carefully checking to avoid complications such as entropion, the incision is closed and the procedure is complete.
