Payment Information

Payments can be made in cash, by credit card, or in installments via medical loans.
If you wish to use a medical loan, please bring a valid ID.

For Minors

Minors under 18 who wish to undergo treatment must either visit the clinic with a parent/guardian or submit a "Parental Consent Form."
*The consent form must be completed by a legal guardian.
